The agency in Sebastian (License #299991610 serves Indian River County. Sebastian River Home Health provides nursing, aides, social workers, physical/occupational/speech therapist. Our home care services are as individualized as the patients we serve.

We are dedicated to caring for your or your loved one 24 hours a day, seven days a week. Our office hours are 8:30 a.m. to 5:00 p.m. Monday through Friday (Except for Holidays).
On Holidays, and after hours, our nursing staff is ON-CALL to meet your needs.

Who Qualifies for Home Health Care?
Under Medicare/Medicaid guidelines, any individual who is “home bound” and needs skilled care on an intermittent basis is eligible to receive services. Other payer's guidelines are subject o the individual policy.
Who Many Request Service?
All services are ordered and approved by a physician to comply with a plan of care. Referrals are usually made at your physician's request. They may also be made by the hospital or on your own.
